Application Intake Guide

Welcome to the TRB Therapist Application system. This form is designed to collect practitioner details. Every submission is automatically recorded in our secure Baserow database and assigned a unique TRB Accreditation Number

How to Use This Form

✅Click the application link provided on the homepage.

✅Complete all required fields with accuracy and care using the information provided by the clients form via email.

✅Once submitted, the application will appear in our internal grid view for review and tier allocation.

Accreditation Number System

Each applicant is assigned a TRB Accreditation Number upon submission. This number reflects their position in the intake list, the year of application, and their current review status.

Format:

Format: TRB-(Year)-(Sequential Number)-(Status Code)

Example:

TRB-2025-001-T4

This indicates:

TRB

2025: Year of application

001: First applicant in the list

T4: Allocated to Tier 4

Status Codes:

P = Pending (awaiting review)

T1 to T4 = Tier allocation (based on practitioner experience, public benefit, and emotional safety flags

Staff may update the status code manually during review
